Dental Lab Remake Request Guide
The information that makes a remake review fast and fair.
Start with the prescription
Include the original prescription: tooth number, material, shade, restoration type and any special instructions. Most remake questions resolve faster when the original intent is clear.
Describe the issue precisely
State what is wrong in concrete terms — marginal gap, contact strength, shade mismatch, occlusal height, or fracture. “Does not fit” is harder to act on than a specific observation.
Provide visual evidence
Photos of the seated restoration, the preparation and the opposing dentition help enormously. Where possible, include an intraoral photo and a photo of the restoration off the model.
Include the records
Provide the original scan or model, bite registration and opposing arch. If the case was digital, resending the files is usually the fastest route.
Note any clinical adjustment
If the restoration was adjusted in the mouth, say so and describe what was adjusted. This affects how the issue is reviewed.
Keep patient information anonymous
Use a case reference rather than patient identifiers. For privacy reasons, avoid sending patient names or personal data.
What happens next
The prescription, submitted files, production information and finished restoration are reviewed together to determine an appropriate repair or remake solution.
